Axentri 150mg Tablet

Axentri contains its active substance called Maraviroc. Maraviroc is an antiretroviral medicine used in the treatment of Human Immunodeficiency Virus (HIV)-1 infection. Maraviroc is a CCR5 co-receptor antagonist. Maraviroc works by acting on the CCR5 receptor which is required for HIV active particles multiplication process thus reduces the viral load of HIV in your body. Maraviroc is used in combination with other antiretroviral HIV medications.

Axentri is used to treat adults, neonates and children who are infected by HIV. Maraviroc must be taken in combination with other anti-HIV medicines. Your doctor will discuss which combination of medicines is best for you.

Axentri is used with other anti-HIV medications to help control HIV infection and helps to decrease the amount of HIV in your body thus your immunity works better by reducing your chance of getting HIV complications like co-infections and other cancers. Maraviroc is a CCR5 receptor antagonist. It is not a cure for HIV infection, it improves immunity that results in longevity of life; thus, precautions are necessary while sexual intercourse, surgery etc.

HIV infection:

HIV infection is a life-threatening condition which attacks our body's immune system and it is a communicable disease that spreads from an infected person through blood and sexual contact and it also spreads from infected mother to fetus during pregnancy or breastfeeding. It is caused by a virus called the human immunodeficiency virus if it is untreated it may lead to AIDS. Symptoms include fever, headache, rash, sore throat, inflamed lymph nodes, diarrhoea, weight loss, cough, night sweats, muscle, and joint pain, pneumonia, thrush, and herpes zoster

It is indicated in the case of HIV infection that causes AIDS in humans. Maraviroc is used with other antiretroviral medications to boost the immunity of the affected hosts by reducing the viral load of the infected cells. It is indicated in neonates, children and adults.

Axentri ( Maraviroc) is a Selective antagonist to the interaction between the human CCR5 receptor and HIV-1 gp120. It blocks this interaction prevents CCR5-tropic HIV-1 entry into cells. Thus, the active HIV molecule becomes inactive and further reproduction and multiplication process is stopped.

Axentri is a CCR5 antagonist. The active component of Maraviroc prevents the interaction between the CCR5 receptor and HIV 1 GP 120 thus prevents entry, multiplication and spread of infection.

Common side effects that occur 1:10 patients: Neutropenia, Thrombocytopenia, increase in hepatic transaminases and bilirubin, Myalgia, Back pain, Weakness

Uncommon side effects that occur in 1:100 patients: Headache, Chills/Fever, Malaise, Insomnia, Anxiety, Depression, Pain, Rash, Nausea, Vomiting, Diarrhea, Pancreatitis, Abdominal pain, Peripheral neuropathy

Rare side effects that occur 1:1,000 patients: pancreatitis, cardiomyopathy, convulsions, rhabdomyolysis, lipodystrophy, lactic acidosis, hepatitis

Very rare side effects that occur 1:10,000 patients: erythema multiform, Stevens-Johnson syndrome, toxic epidermal necrolysis, lactic acidosis, pure red cell aplasia, hepatic failure

• Is Axentri (Maraviroc ) safe during pregnancy?

This medication is recommended in pregnancy. Consult your physician.

• Can Axentri (Maraviroc ) be taken while breast feeding?

There is no specific study to detail this. But in general HIV virions can pass via breastfeeding thus it is suspected that antiretrovirals can also pass via breastfeeding. Thus, consult your physician.

• Can Axentri (Maraviroc ) be taken while alcohol consumption?

The interaction is unknown but this medication causes side effects. Thus, consult your physician.

• Can I take a drive or operate heavy machinery while on Axentri (Maraviroc )?

Alcohol

Pregnancy

Breast Feeding

• The use of Maraviroc during breastfeeding is not recommended as evidence shows less amount of the medication passes via breastfeeding to the baby that may cause adverse effects in newborn.

Kidney Problem

• Axentri 150mg Tablet should be used with dose adjustment and caution for the patient with kidney problems. Kindly consult with your doctor.

Liver Disease

• Inform your physician if you have hepatitis B or C or fatty liver or Childs Pugh disease as it causes hepatotoxicity

• It should be tablet is taken orally with or without food.
• Axentri should be taken at a specific time
• Don’t chew or break the tablet.
• Swallow the whole tablet fully.
• It must be taken with other antiretrovirals.
• After therapy has been initiated, do not take more or less of this drug than prescribed or stop taking it without your physicians’ approval. Skipping or changing your dose without approval would increase the viral load making the infection more difficult to treat, or worsen side effects. • Diabetic
• Renal disorders
• Any sign of infection – fever or lymph node enlargement
• Musculoskeletal problems - Osteonecrosis that causes joint stiffness and pain in larger joints like shoulder, hip, and knee
• Skin reactions - It can be mild to severe like Stevens-Johnson Syndrome and toxic epidermal necrolysis.
• Accumulation or Redistribution of body fat or  immune reconstitution syndrome

• If you have any allergy/ hypersensititvity

• If you are pregnant or breastfeeding

Maraviroc is taken with other antiretrovirals orally at a prescribed dosage at a specific time regularly with food. If Maraviroc is taken separately the efficiency of the drug is reduced significantly, thus it is ill-advised.

Drug-Drug Interaction

Thioridazine + Maraviroc

The use of this drug is contraindicated because the use of maraviroc will increase the effect of thioridazine by affecting the patient hepatic enzyme metabolism.

Lasmiditan + Maraviroc

Use alternative drugs because the use of lasmiditan increases the level of maraviroc by p-glycoprotein efflux transporter.

Acebutolol + Maraviroc

Therapy should be administered with caution because the combination of this drug either increases the effect of the other by pharmacodynamic synergism and it also increased the risk of orthostatic hypotension

Amiodarone + Maraviroc

Therapy should be administered with caution because the use of amiodarone will increase the effect of maraviroc by P-glycoprotein efflux transporter.

Aprepitant + Maraviroc

 Therapy should be administered with caution because the use of aprepitant will increase the effect of maraviroc by affecting the patient hepatic or intestinal enzyme metabolism

Drug-Food Interaction

Consuming high-fat meals may decrease the bioavailability of maraviroc. The drug can be taken with or without as per the dose advised by the doctor.Kindly consult with your doctor

Drug-Disease Interaction

Liver impairment:

Therapy should be administered with caution in patients with hepatic impairment because the use of this drug may cause hepatotoxicity and cause a severe allergic reaction like fever, eosinophilia, increase immunoglobulin E. Before initiating the treatment monitor the patients liver enzymes level.

Renal impairment:

Therapy should be administered with caution in patients with severe renal impairment and end-stage renal disease because the use of this drug may increase the risk of developing postural hypotension. Adjust the dose if you have any symptoms of postural hypotension.

Cardiovascular diseases:

Therapy should be administered with caution in a patient with cardiovascular diseases because the use decrease the blood pressure and also increase the risk for developing cardiovascular adverse effects

Symptoms of overdose include dizziness, palpitations etc. Contact your doctor immediately.

You can take the missed dosage if you notice within 6 hours if you take them twice a day. If it is more than 6 hours, then skip the dosage and take the next doses as usual. Always take with other antiretrovirals and food Don’t take a double dose for a forgotten dose
