Mechanism of action of Barec AC Capsule SR
Barec AC Capsule SR contains Aceclofenac (200mg) and Rabeprazole (20mg). Aceclofenac is an analgesic and anti-inflammatory medication. It produces its action by inhibiting the cyclooxygenase (COX-1) and (COX-2). Thus it inhibits the synthesis of prostaglandins which is responsible for pain and inflammation. It also inhibits the aggregation and activation of neutrophil and chemotaxis, decreases the proinflammatory cytokine level, and alter the activity of lymphocyte.
Rabeprazole is a proton pump inhibitor. It produces its action by inhibiting the H+/K+ ATP pumps which are released by parietal cells in the stomach for gastric acid secretion. Blocking these pumps reduces the acid secretion in the stomach and helps to get relief from the acidity. This prevents the damage of the stomach lining caused by Aceclofenac.

Apixaban
The use of this drug is contraindicated because the combination of this drug increases the effect of anticoagulation
Captopril

Aliskiren

Methotrexate
Therapy should be administered with caution because the use of rabeprazole increases the effect of methotrexate by decreasing its renal clearance. At a higher dose, it increases the risk of developing toxicity.

Hypertension
Therapy should be administered with caution in patients with hypertension because the use of NSAIDs may increase blood pressure and cardiovascular events. During this course of treatment monitor the patient blood pressure
Renal toxicities
Therapy should be administered with caution in a patient with renal diseases because the use of NSAIDs may increase the serum creatinine, BUN, tubular necrosis, glomerulus, renal papillary necrosis, nephrotic syndrome, and cause renal failure.
