    Medical Description of Dinmek Tablet

    Dinmek tablet contains Tenofovir disoproxil fumarate 300mg, Lamivudine 300mg, and Efavirenz 600mg. Dinmek (Lamivudine (300mg) + Tenofovir disoproxil fumarate (300mg) + Efavirenz (600mg)) tablet is an antiretroviral medicine used in the treatment of Human Immunodeficiency Virus (HIV)-1 infection. Dinmek tablet in which Tenofovir disoproxil fumarate and Lamivudine is a nucleoside reverse transcriptase inhibitor and Efavirenz is a non-nucleoside reverse transcriptase inhibitor. Dinmek tablet is used adults and adolescence above 35Kgs.


    Dinmek Tablet Uses

    Dinmek (Lamivudine (300mg) + Tenofovir disoproxil fumarate (300mg) + Efavirenz (600mg)) tablet is used to treat adults and adolescence above 35Kgs of weight who are infected by HIV. It is used in caution with hepatitis B.

    Dinmek Tablet Indication

    Dinmek (Lamivudine (300mg) + Tenofovir disoproxil fumarate (300mg) + Efavirenz (600mg)) tablet is indicated in the treatment of HIV infection that causes AIDS in humans. Dinmek tablet is used to boost the immunity of the affected hosts by reducing the viral load of the infected cells. Prior to administration of Dinmek tablet screening for hepatitis B infection must be done. Dinmek tablet is indicated in weight above 35 Kgs.


    Mechanism of action of Dinmek Tablet

    Dinmek (Lamivudine (300mg) + Tenofovir disoproxil fumarate (300mg) + Efavirenz (600mg)) tablet is which Tenofovir disoproxil fumarate and Lamivudine are nucleoside analogue that acts as a reverse transcriptase inhibitor and Efavirenz is a non-nucleoside reverse transcriptase inhibitor. Tenofovir disoproxil fumarate is an analogue of adenosine 5-monophosphate which is converted to tenofovir after hydrolysis after which tenofovir inhibits HIV-1 reverse transcriptase. Tenofovir competes with AMP as a substrate and competes to incorporate themselves into the DNA strand thus stops the replication of viral particles.  Lamivudine is an analogue that is phosphorylated to its active forms lamivudine triphosphate that is incorporated into the nucleotide and inhibits the action of HIV-1 reverse transcriptase. Efavirenz is a non-nucleoside reverse transcriptase that blocks RNA and DNA dependant polymerase in HIV replication. Thus, the active HIV molecule becomes inactive and further reproduction and multiplication process is stopped.


    How Dinmek Tablet works

    Dinmek (Lamivudine (300mg) + Tenofovir disoproxil fumarate (300mg) + Efavirenz (600mg)) tablet has two nucleoside analogue that acts as reverse transcriptase inhibitor and one non-nucleoside reverse transcriptase inhibitor. The active substance of tenofovir and lamivudine competes with AMP and incorporates themselves into DNA strand and thus replication process of viral particle is terminated. The active substance of Efavirenz DNA and RNA dependant polymerase blocks HIV replication. Thus, the active HIV molecule becomes inactive and further reproduction and multiplication process is stopped.


    Dinmek Tablet Contraindication

    Absolute contraindication:

    • if you are allergic to Dinmek (Lamivudine (300mg) + Tenofovir disoproxil fumarate (300mg) + Efavirenz (600mg)) tablet and its contents
    • if <18 years of age
    • if you are breastfeeding
    • if you are pregnant

    Relative contraindication:

    • if you have liver disease like hepatitis B or C or hepatomegaly with steatosis
    • if you have kidney impairment
    • if you have any heart disease


    Dinmek Tablet Side Effects

    Common side effects that occur 1:10 patients: Headache, insomnia, Cough, Nausea, vomiting, abdominal pain or cramps, diarrhoea, Rash, alopecia, Arthralgia, Fatigue, malaise, fever, hypophosphataemia, asthenia, Neutropenia, anaemia, thrombocytopenia

    Uncommon side effects that occur 1:100 patients: Neutropenia and anaemia, thrombocytopenia, hypokalaemia, proximal renal tubulopathy, elevation in AST and ALT levels 

    Rare side effects that occur 1:1,000 patients: Pancreatitis, Hepatitis, Angioedema, hepatic steatosis, osteomalacia, acute renal failure, renal failure, acute tubular necrosis, nephritis 2, nephrogenic diabetes insipidus,  

    Very rare side effects that occur 1:10,000 patients: Pure red cell aplasia, Lactic acidosis, Peripheral neuropathy, Rhabdomyolysis

    How to Take Dinmek Tablet

    Dinmek (Lamivudine (300mg) + Tenofovir disoproxil fumarate (300mg) + Efavirenz (600mg)) tablet is taken orally without food.

    Dinmek tablet should be taken at a specific time every day.

    Dinmek tablet is best taken at bedtime.

    Don’t chew or break the tablet.

    Swallow Dinmek tablet as whole.

    After therapy has been initiated, do not take more or less of Dinmek tablet drug than prescribed or stop taking it without your physicians’ approval. Skipping or changing Dinmek tablet dose without approval would increase the viral load making the infection more difficult to treat, or worsen side effects.     Use of Dinmek Tablet in Elderly Patients

    Dinmek (Lamivudine (300mg) + Tenofovir disoproxil fumarate (300mg) + Efavirenz (600mg)) tablet has only been used in few patients 65 years or older. Dosage and usage are similar to adults. Discuss with your physician before usage.


    Other general warnings

    • Blood disorders – neutropenia, anemia etc.

    Over Dose

    Missed Dose

    You can take the missed dosage if you notice within 12 hours if you take them once a day. If it is more than 12 hours, then skip the dosage and take the next doses as usual. Don’t take a double dose for a forgotten dose.
