Emduo N (Lamivudine (150mg) + Stavudine (30mg) + Nevirapine (200mg)) Kit is an antiviral medicine used in the treatment of Human Immunodeficiency Virus (HIV) – 1 infection. Emduo N Kit is a nucleoside analogue. Emduo N Kit works by reducing the viral load of HIV in your body. Emduo N Kit improves your immune system and reduces the risk of conditions associated with HIV infection.

Emduo N (Lamivudine (150mg) + Stavudine (30mg) + Nevirapine (200mg)) Kit is used to treat adults and adolescence >50 kgs of weight or >12 years of age who are infected by HIV-1. Caution is used in cases of liver and renal impairment.

Emduo N (Lamivudine (150mg) + Stavudine (30mg) + Nevirapine (200mg)) Kit is used with other anti-HIV medication to help control HIV infection. Emduo N Kit helps to decrease the amount of HIV in your body thus your immunity works better by reducing your chance of getting HIV complications like co-infections and other cancers. Emduo N Kit is a nucleoside analogue. Emduo N Kit is not a cure for HIV infection, it improves immunity that results in longevity of life. It is not recommended for use in children less than 12 years of age or less than 50 kgs of weight due to the increased risk of serious side effects.

Emduo N (Lamivudine (150mg) + Stavudine (30mg) + Nevirapine (200mg)) Kit is indicated in the treatment of HIV-1 infection that causes AIDS in humans. Emduo N Kit is used to boost the immunity of the affected hosts. Emduo N Kit is indicated in ages above 12 years of age who weigh more than 50 Kgs.

Emduo N Kit contains Lamivudine 150mg, stavudine 30mg and nevirapine 200mg. Lamivudine is HIV-1 nucleoside reverse transcriptase inhibitor. Lamivudine is phosphorylated to its active forms lamivudine triphosphate that is incorporated into the nucleotide and inhibits the action of HIV-1 reverse transcriptase. Stavudine is a nucleoside reverse transcriptase inhibitor that is phosphorylated into stavudine -5- triphosphate. Stavudine-5-triphosphate battle with deoxythymidine triphosphate and incorporate themselves into the DNA of HIV molecular structure. After they incorporate within the DNA of HIV molecule the further process such as transcription, reverse transcriptase enzyme is inhibited. Nevirapine is a non-nucleoside reverse transcriptase inhibitor that binds to reverse transcriptase of HIV-1 particle thus blocks RNA and DNA dependant DNA polymerase actions that is required for replication of HIV-1. Thus, the active HIV molecule becomes inactive and further reproduction and multiplication process is stopped.

Emduo N (Lamivudine (150mg) + Stavudine (30mg) + Nevirapine (200mg)) Kit is HIV-1 nucleoside reverse transcriptase inhibitor. Emduo N Kit inhibits the action of HIV-1 reverse transcriptase enzyme by incorporating themselves into the nucleotide sequence thus inhibits the action of the reverse transcriptase of HIV-1, thereby preventing the formation of mature virus particles into infectious virions that spreads the infection and improves immunity.

Absolute contraindication:

• if you are allergic to Emduo N (Lamivudine (150mg) + Stavudine (30mg) + Nevirapine (200mg)) Kit and its contents
• if you are pregnant
• if you are breastfeeding
• children <12 years of age.

Relative contraindication:

• if you have a liver disorder like hepatitis B or C or Child-Pugh disease
• if you have neurologic disorders
• if you have pancreatitis
• if you have renal diseases or filtration impairment

Very common side effects that occur in more than 1:10 patients: rash

Common side effects that occur 1:10 patients: Headache, Cough, nausea, vomiting, abdominal pain, diarrhea, alopecia, Arthralgia, Myalgia, Fatigue, malaise, fever, Back pain. Neutropenia, Thrombocytopenia, increase in AST, ALT bilirubin levels, granulocytopenia, hepatitis

Uncommon side effects that occur 1:100 patients: anemia, Insomnia, Anxiety, Depression, Pancreatitis, Peripheral neuropathy, difficulty breathing, anaphylactic shock, jaundice, Stevens-Johnson Syndrome/toxic epidermal necrolysis, urticaria, angioedema, arthralgia, myalgia, decreased blood phosphorus, increased blood pressure

Rare side effects that occur 1:1000 patients: Pure red cell aplasia, Lactic acidosis, Pancreatitis, elevations in serum amylase, Angioedema, Rhabdomyolysis, cardiomyopathy, convulsions, lipodystrophy, lactic acidosis, fulminant hepatitis, drug reaction with eosinophilia, decrease in skin sensations

The use of Emduo N (Lamivudine (150mg) + Stavudine (30mg) + Nevirapine (200mg)) Kit during breastfeeding is not recommended as evidence shows less amount of the medication passes via breastfeeding to the baby that may cause adverse effects in newborn.

Kidney Problem

Renal filtration impairment – monitor function of creatinine clearance rate before and during treatment. caution is used in case creatinine clearance is <50ml/min.

Liver Disease

Liver disorders and diseases like hepatitis B or C, hepatic steatosis

Monitor liver function like AST, ALT and bilirubin levels before and during therapy

Emduo N (Lamivudine (150mg) + Stavudine (30mg) + Nevirapine (200mg)) Kit is taken orally without food.

Emduo N Kit must be taken at the same time every day as prescribed by your physician.

After therapy has been initiated, do not take more or less of Emduo N medicine than prescribed or stop taking it without your physicians’ approval. Skipping or changing Emduo N Kit dose without approval may cause the amount of the virus to increase making the infection more difficult to treat, or worsen side effects.

If you are pregnant or breastfeeding inform your physician as they are not advisable.

The lead-in period for Emduo N Kit to build tolerance is 14 days.

The recommended dosage is one tablet of Emduo N Kit taken once a day followed by an oral dose of Lamivudine 150mg and Stavudine 30mg after 12 hours.

After the lead-in period the tolerance is checked (skin rash occurrence/ liver and kidney/ blood workup) the recommended maintenance dosage for Emduo N Kit taken twice a day orally

Emduo N Kit should be taken as prescribed by the physician without fail. These medicines must be taken without food on a regular time basis either 2 hours before food or 2 hours after food intake

Emduo N (Lamivudine (150mg) + Stavudine (30mg) + Nevirapine (200mg)) Kit has only been used in few patients 65 years or older. Dosage and usage are similar to adults. Discuss with your physician before usage.

• Pancreatitis

Drug-Drug Interaction

Serum concentration decreased by Acetazolamide, Potassium citrate

Serum concentration increased by Aceclofenac, Acetaminophen, Ketoconazole, Itraconazole

Severity of adverse effects is increased by Acyclovir, Dopamine, Histamine, Metformin

Drug-Food Interaction

Emduo N (Lamivudine (150mg) + Stavudine (30mg) + Nevirapine (200mg)) Kit can be safely taken with any type of food, if doubts arise kindly refer to your physician. It is best to take Emduo N Kit on an empty stomach either 2 hrs before or after food. Emduo N Kit generally causes side effects thus it is advised not to be taken with alcohol.

Drug-Disease Interaction

• Liver disorders – used with caution in severe liver diseases. Consult a physician before use

• Cardiac disorders – used with caution in heart failure and MI. Consult a physician before use

Contact your doctor, pharmacist or nurse immediately.

If you take twice a day dosage, then the missed dosage if you notice within 6 hours. If it is more than 6 hours, then skip the dosage and take the next doses as usual. Don’t take a double dose for a forgotten dose.
