Hifenac-TL tablet contains Aceclofenac 100mg and Tramadol 37.5mg. Aceclofenac is an analgesic and anti-inflammatory medication. It produces its action by inhibiting the cyclooxygenase (COX-1) and (COX-2). Thus it inhibits the synthesis of prostaglandins which is responsible for pain and inflammation. It also inhibits the aggregation and activation of neutrophil and chemotaxis, decreases the proinflammatory cytokine level, and alter the activity of lymphocyte.
Tramadol is a centrally acting analgesic that produces its action by binding to the opioid mu receptors and causing inhibition of ascending pain pathway and relieve pain.

Therapy should be administered with caution in patients with infectious diarrhea due to pseudomembranous enterocolitis. The use of opioids may decrease gastrointestinal motility which may delay the excretion of infective gastroenteric organisms.
Liver diseases + Tramadol
Therapy should be administered with caution in patients with hepatic impairment because the use of opioid analgesic may increase the serum concentration of the metabolite and the half-lives prolonged in patients with impaired hepatic functions.
Adrenal insufficiency + Tramadol
Therapy should be administered with caution in patients with adrenocortical insufficiency. Patients with Addison’s disease may have an increased risk of respiratory depression and CNS depression.
