Viraday Tablet

Viraday tablet contains Emtricitabine 200mg, Tenofovir disoproxil fumarate 300mg and Efavirenz 600mg. Viraday (Emtricitabine (200mg) + Tenofovir disoproxil fumarate (300mg) + Efavirenz (600mg)) tablet is an antiretroviral medicine used in the treatment of Human Immunodeficiency Virus (HIV)-1 infection. Viraday tablet has two nucleoside reverse transcriptase inhibitor and one non-nucleoside reverse transcriptase inhibitor. Viraday tablet acts on HIV reverse transcriptase enzyme thus interferes with their reproduction cycle.

Viraday (Emtricitabine (200mg) + Tenofovir disoproxil fumarate (300mg) + Efavirenz (600mg)) tablet is used to treat adults and children above 12 years of age or weigh more than 40 kgs who are infected by HIV-1.

Viraday (Emtricitabine (200mg) + Tenofovir disoproxil fumarate (300mg) + Efavirenz (600mg)) tablet is used with other anti-HIV medication to help control HIV infection and helps to decrease the amount of HIV virus in your body thus your immunity works better by reducing your chance of getting HIV complications like co-infections and other cancers. It is not a cure for HIV infection, it improves immunity that results in longevity of life; thus, precautions are necessary while sexual intercourse, surgery etc. it is safely used in children above 12 years of age or weigh more than 40 Kgs.

Viraday (Emtricitabine (200mg) + Tenofovir disoproxil fumarate (300mg) + Efavirenz (600mg)) tablet is indicated in case of HIV infection that causes AIDS in humans. Viraday tablet is used to boost immunity of the affected hosts by reducing the viral load of the infected cells. Viraday tablet is safely used in children above age 12 or weigh more than 40 Kgs.

Viraday (Emtricitabine (200mg) + Tenofovir disoproxil fumarate (300mg) + Efavirenz (600mg)) tablet has two nucleoside reverse transcriptase inhibitors and one non-nucleoside reverse transcriptase inhibitor. Tenofovir disoproxil fumarate is an analogue of adenosine 5-monophosphate which is converted to tenofovir after hydrolysis after which tenofovir inhibits HIV-1 reverse transcriptase and HBV reverse transcriptase. Tenofovir competes with AMP as a substrate and competes to incorporate themselves into the DNA strand thus stops the replication of viral particles. Emtricibine is an analogue of cytidine is phosphorylated to its active form Emtricibine 5 triphosphate that inhibits the activity of HIV-1 reverse transcriptase and incorporates themselves into the DNA thus results in termination of replication process. Efavirenz present is a non-competitively inhibits HIV-1 reverse transcriptase. Thus, the active HIV molecule becomes inactive and further reproduction and multiplication process is stopped by the use of Viraday tablet.

Viraday tablet contains Emtricitabine 200mg, Tenofovir disoproxil fumarate 300mg and Efavirenz 600mg. The active substance of tenofovir and Emtricibine competes with AMP and cytidine respectively and incorporates themselves into DNA strand and thus replication process of viral particle is terminated. The active substance of Efavirenz non competitively inhibits the reverse transcriptase enzyme of HIV particles. Thus, the active HIV molecule becomes inactive and further reproduction and multiplication process is stopped.

Absolute contraindication:

• if you are allergic to Viraday (Emtricitabine (200mg) + Tenofovir disoproxil fumarate (300mg) + Efavirenz (600mg)) tablet and its components
• if <40 kgs of weight
• if age is <12 years
• if you are breastfeeding
• if you are pregnant

Relative contraindications:

• if you have liver disease like hepatitis B or C or hepatomegaly with steatosis
• if you have kidney impairment
• if you have heart disease or disorder

Common side effects that occur 1:10 patients: neutropenia, hypophosphatemia, hypertriglyceridemia, hyperglycaemia, hypertriglyceridemia, allergic reaction, depression, anxiety, abnormal dreams, insomnia, abnormal dreams, insomnia, headache, dizziness, cerebellar coordination and balance disturbances, diarrhoea, nausea, elevated creatine kinase

Uncommon side effects that occur 1:100 patients: anaemia, hypersensitivity, hypercholesterolaemia, hypokalaemia, suicide ideation, psychosis, mania, paranoia, hallucination, convulsions, amnesia, ataxia, tremor, tinnitus, vertigo, flushing, Stevens-Johnson syndrome, erythema multiforme, angioedema, gynaecomastia,

Rare side effects that occur 1:1,000 patients: Lactic acidosis, delusion, neurosis, pancreatitis, osteomalacia, renal failure, acute tubular necrosis, nephritis, nephrogenic diabetes insipidus, hepatic steatosis, hepatitis, hepatic failure

Is Viraday (Emtricitabine (200mg) + Tenofovir disoproxil fumarate (300mg) + Efavirenz (600mg)) tablet safe during pregnancy?

This medication is not recommended in pregnancy unless pros outweigh the cons. Consult your physician.

Can Viraday tablet be taken while breastfeeding?

There is no specific study to detail this. But in general HIV virions can pass via breastfeeding thus it is suspected that antiretrovirals can also pass via breastfeeding. Thus, consult your physician.

Can Viraday tablet be taken while alcohol consumption?

The interaction is unknown but this medication causes dizziness. Thus, consult your physician.

Can I take a drive or operate heavy machineries while on Viraday tablet?

This medication causes dizziness and instability thus caution is used while driving or operating machineries.

Can I take supplements with Viraday tablet?

This medication needs to be taken on an empty stomach thus avoid taking any other medications like supplements with Viraday tablet but it is advised that you can safely take this medication at 12 hrs apart.

Alcohol

Breast Feeding

The use of Viraday (Emtricitabine (200mg) + Tenofovir disoproxil fumarate (300mg) + Efavirenz (600mg)) tablet during breastfeeding is not recommended as evidence shows less amount of the medication passes via breastfeeding to the baby that may cause an adverse effect in newborn.

Kidney Problem

Inform your physician if you have any renal impairment as this treatment may cause added effects on kidney thus worsens the condition.

Liver Disease

Inform your physician if you have hepatitis B or C

Viraday tablet should be taken at a specific time every day.

Don’t chew or break the tablet.

Swallow Viraday tablet as a whole.

After therapy has been initiated, do not take more or less of Viraday tablet than prescribed or stop taking it without your physicians’ approval. Skipping or changing Viraday tablet dose without approval would increase the viral load making the infection more difficult to treat, or worsen side effects. • Lactic acidosis
• Bone density disorders - osteomalacia
• Heart diseases or disorders – MI
• Musculoskeletal problems - Osteonecrosis that causes joint stiffness and pain in larger joints like shoulder, hip, and knee
• Accumulation or Redistribution of body fat or immune reconstitution syndrome.

Viraday (Emtricitabine (200mg) + Tenofovir disoproxil fumarate (300mg) + Efavirenz (600mg)) tablet is taken orally at prescribed dosage at specific time regularly with food. If Viraday tablet is taken without food the efficiency of drug is reduced significantly, thus it is ill advised.

Drug-Drug Interaction

Adverse effects are increased by aceclofenac, acyclovir, amikacin, Ganciclovir, acetylcholine

Serum concentration is decreased by Acetazolamide, carbamazepine, flunarizine

Serum concentration is increased by Carvedilol, amiodarone, clomifene, acetaminophen, live vaccines

Drug-Food Interaction

Viraday (Emtricitabine (200mg) + Tenofovir disoproxil fumarate (300mg) + Efavirenz (600mg)) tablet should be taken on an empty stomach. Intake of food is best after 2 hrs to intake of food, if doubts arise kindly refer to your physician. It generally causes known side effects thus it is advised not to be taken with alcohol.

Drug-Disease Interaction

• Liver disorders – used with caution in severe liver diseases. Consult a physician before use
• Kidney disorders – used with caution in severe kidney diseases. Consult a physician before use
• Cardiac disorders – used with caution in heart failure and MI. Consult a physician before use
• Respiratory disorders – used with caution in respiratory distress. Consult a physician before use

Symptoms of overdose include dizziness, palpitations etc. Contact your doctor immediately.

You can take the missed dosage if you notice within 12 hours if you take them once a day. If it is more than 12 hours, then skip the dosage and take the next doses as usual. Always take with food. Don’t take a double dose for a forgotten dose.
